Agreements and waivers

Step 2 of 3

Waiver of liability and assumption of risk Slo-pitch carries risk of injury including collisions, thrown and batted balls, and uneven field conditions. I assume those risks and release the league, its volunteers, and the Town of Milton from claims arising from participation.

Insurance acknowledgment The league carries general liability coverage through its provincial association. That coverage does not include accident, medical, or dental benefits for players. I confirm I carry my own health coverage.

Code of conduct I have read the code of conduct and accept that ejections carry an automatic suspension.

Photo consent I consent to team and action photos being used on league channels. Untick this and no photograph of you will be published. This one is optional — it does not stop you registering.
